Using Repeatable Sections to Collect Information for Multiple People

Custom Forms can collect information for more than one person by using repeatable sections.

This is helpful when one person is completing a form on behalf of multiple people, such as a parent submitting information for children, a household contact submitting information for family members, or a leader submitting information for multiple participants.


What is a section?

A section is a group of fields on a Custom Form.

Sections help organize your form into related areas, such as:

  • Personal Information
  • Child Information
  • Volunteer Details
  • Emergency Contact
  • Ministry Interest
  • References

A section can simply organize questions, or it can be used to represent a person.

What is a repeatable section?

A repeatable section allows the person completing the form to add another copy of that section.

For example, if a form has a repeatable section called Child Information, the submitter could enter one child, then add another child, then another.

This allows one form submission to collect information for multiple people without duplicating a bunch of empty fields that may not get used.

Creating a section for another person

In the form builder, open the section settings (by clicking on the name of the section).

To use a section for creating or matching a person record, check:

Create a new person record (or match existing person) based on this section.

This option is for creating or matching an additional person based on the information collected in that section.

If the section should allow the submitter to add more than one person, also check:

This section is repeatable, allowing user to add additional.

Then, under What does this section represent?, enter a label, such as Person. You could also use this to represent something other than people, such as "Item" when collecting a list of items someone wishes to donate for a fundraiser.

The main form setting links the submission to the primary submitter. The section setting allows specific sections to create or match additional people.

Example: Parent submitting information for children

A children’s ministry form might be set up like this:

Primary Submitter Information

  • First Name
  • Last Name
  • Email
  • Cell Phone

Child Information

  • First Name
  • Last Name
  • Birthday
  • Grade
  • Allergies
  • Medical Concerns

The Child Information section could be set as repeatable and marked as representing a person.

This would allow a parent to submit information for more than one child from the same form.

Fields to include in a person section

When a section represents a person, include enough information to help Connection Card Pro identify that person.

Helpful fields may include:

  • First Name
  • Last Name
  • Email, if applicable
  • Cell Phone, if applicable
  • Birthday
  • Grade
  • Address, if needed

For children, email and phone may not always apply, so birthday, grade, and family context may be helpful.

Use preset fields when collecting person information

When adding fields that represent standard person information, use preset fields when possible.

For example, use the Birthday preset for birthday, the Email preset for email, and the Cell Phone preset for cell phone.

Preset fields help Connection Card Pro understand what type of information is being collected.

Avoid unnecessary repeatable person sections

Only use repeatable person sections when the form truly needs to collect information about multiple people.

Do not use a repeatable person section when you only need:

  • A single form response
  • A list of preferences
  • A group of checkbox choices
  • A general comment box
  • A one-person application

Using repeatable person sections unnecessarily may make the form more confusing and may increase the chance of creating duplicate person records.

Best practices

Before sharing a form with repeatable person sections:

  • Test the form yourself.
  • Submit the form as if you are a parent, leader, or applicant.
  • Confirm that each section makes sense.
  • Make sure required fields are truly needed.
  • Check that person fields use the correct presets.
  • Confirm whether the submission links to the correct person.
  • Confirm whether additional people are created or matched as expected.
